Our custom Questionnaire helps you to organize your tax data and is key to ensuring the accuracy and completeness of your tax return. A completed Questionnaire makes it easy to determine the documents and information you will need to bring to our office.

Please review any questions to which you respond, “YES” – these are generally questions that require supporting forms or other documentation. Include the requested forms or documentation with this completed questionnaire when you deliver your tax materials to our office.

Our office is proud of our effort to be as paperless as possible. We scan all original documents you provide us and return them to you with your completed tax packet. Please do not staple any documents as they jam our scanner's auto-feeder.

Note: Several questions reference worksheets. Include the Client Information and Dependent Information worksheets in your completed packet. Please download and fill out any of the other worksheets that you need!

A Quick Tip: when answering questions, unless otherwise noted, “you” refers to the taxpayer and/or the spouse.
